Qualifications

• Master’s in Social Work is required.
• Experience in individual and family assessment, crisis intervention, grief and loss counseling, and discharge planning preferred.
• Master Social Work licensure in Michigan upon hire is required.

Responsibilities

• Assist patients and families with complex psychosocial needs and discharge planning.
• Coordinate with alternate sites of care and community agencies to address patient needs.
• Document assessments and plans in the electronic medical record.
• Provide consultation and resources to the healthcare team.
• Maintain knowledge of community resources.
