Job summary

ChenMed LLC is seeking a Supportive Care RN to manage the clinical and operational aspects of patient engagement for the serious illness management panel. You will perform thorough home or center visits, assess clinical status, and coordinate with palliative and specialty care teams to ensure timely and compassionate care transitions.

Responsibilities include daily outreach, documentation, and escalation when needed.

Qualifications

• Active RN license in the state of practice.
• Minimum 2 years of clinical nursing experience.
• Case management and coordination experience preferred.
• Bilingual abilities not required but helpful.

Responsibilities

• Perform comprehensive home, center, or facility assessments.
• Identify and respond to clinical changes with escalation as needed.
• Coordinate post-discharge care and follow-up visits.
• Maintain documentation of all patient interactions within 24 hours.
• Communicate with PCPs, specialists, and care team to ensure goals of care.
