Job summary

ChenMed in Virginia is seeking an Intensive Community Care Manager (RN) to lead the Complex Care Team for high-risk patients, coordinating care plans with PCPs and clinicians to prevent unnecessary hospital visits and keep patients safely at home.

You will perform home visits, assess needs, develop comprehensive care plans, provide supervision to team members, and ensure smooth transitions of care. Strong communication and clinical leadership are essential.

Qualifications

• Associate degree in Nursing required.
• Bachelor’s Degree in nursing (BSN) or RN with bachelor’s degree preferred.
• A valid, active RN license in the state of employment (compact license preferred).
• Minimum 2 years of clinical work experience required.
• Minimum 1 year of case management experience preferred.
• BLS certification required within 90 days of employment.

Responsibilities

• Provides in-house, at facility, and telephonic visits to high-risk patients to prevent unnecessary hospital admissions.
• Provides home visits and develops care plans to address goals and barriers.
• Conducts supervisory visits with LPN and patient to oversee discharge from case management.
• Performs SD (Social Determinants of Health) assessments and health monitoring in outpatient and home settings.
• Coordinates, reports, and follows-up on multidisciplinary team meetings and care plans.
