Qualifications

• Active RN license in the state of practice.
• Minimum 2 years of clinical nursing experience.
• Experience in case management or palliative care preferred.
• Willingness to travel locally up to 75%.

Responsibilities

• Identify patients, outreach, and enrollment with rapid first contact.
• Perform home or center-based comprehensive assessments.
• Coordinate care with PCPs, specialists, and hospice partners.
• Document visits and scoring tools (ESAS-r, PPS, FRAIL, ECOG).
