Job summary

The incumbent will advocate for patients, coordinate social services, and support discharge planning while maintaining regulatory compliance and quality care standards.

Qualifications

• RN license in the state of employment is required.
• Associate degree in Nursing required; BSN preferred.
• Minimum 2 years clinical work experience.
• Minimum 1 year in community case management preferred.
• Valid driver's license required.

Responsibilities

• Coordinate integration of case management into patient care, discharge and home planning.
• Conduct concurrent medical record reviews against approved indicators.
• Advocate for patients; educate staff on resources and discharge planning.
• Promote efficient clinical resource utilization toward outcomes within set timeframes.
• Review service utilization from admission through discharge.
• Assess patient satisfaction and quality of care provided.
• Communicate with physicians and develop collaborative relationships.
• Develop and implement patient care policies and protocols; assist in complex cases.
• Coordinate social services for patients and families to maximise benefits.
