Job summary

ChenMed is seeking a Supportive Care, RN to provide comprehensive palliative care for seriously ill patients. This role involves home or center-based assessments, building trust with patients, and coordinating with PCPs and specialists to align goals of care.

You will monitor symptoms, document outcomes, and assist families through transitions to hospice when appropriate. Willingness to travel within the market and flexible hours, including evenings, is required.

Qualifications

• Active RN license in the state of practice.
• Minimum 2 years clinical nursing experience.
• Case management or oncology/palliative care experience preferred.
• Bilingual preferred if applicable.

Responsibilities

• Identify and enroll Tier 1/2 patients within 48 hours.
• Perform comprehensive home or center-based assessments.
• Coordinate scheduling with care teams and obtain consent.
• Monitor patient status and escalate when needed.
• Document all patient contacts within 24 hours.
• Coordinate transitions to hospice when appropriate.
