Job summary

ChenMed is seeking a Supportive Care RN to deliver comprehensive palliative and seriously ill patient care across home and center settings in the Lakeland area. You will perform in-home assessments, monitor symptoms, and collaborate with PCPs, oncologists and specialists to coordinate care plans.

Responsibilities include initiating outreach within 48 hours of referrals, documenting goals of care, managing pain and medications, and ensuring timely hospice transitions when appropriate.

Qualifications

• Active RN license in the state of practice.
• ADN minimum; BSN preferred.
• Minimum 2 years of clinical nursing experience.
• Case management, oncology, cardiology, pulmonology, nephrology, home health, or hospice/palliative care experience preferred.

Responsibilities

• Identify and enroll patients within 48 hours of Tier 1/2 signals and referrals.
• Conduct head-to-toe clinical assessments in home, center, or facility.
• Coordinate scheduling with palliative care and specialty teams.
• Document goals of care and advance directives at visits.
• Communicate changes to PCP and specialists promptly.
