ChenMed in Philadelphia seeks an Intensive Community Care Manager (RN) to lead field nursing and home-based care for our highest-risk patients. You will coordinate with PCPs, design care plans, and supervise team members to keep patients safely at home and reduce hospitalizations.

The role is on-site with regular home visits, requiring strong clinical judgment, communication, and autonomy. Travel up to 10% is expected. ChenMed offers competitive benefits and growth opportunities.

Qualificações

• Active RN license in the relevant state.
• Minimum 2 years of clinical or case management experience.
• Ability to travel locally and regionally up to 10%.

Responsabilidades

• Provide in-house, facility, and telephonic visits to high-risk patients to prevent hospital admissions.
• Develop and coordinate individualized care plans with PCPs.
• Supervise LPNs and other team members as needed.
• Monitor patient progress and adjust care plans accordingly.
