Qualifications

• Associate degree in Nursing required.
• RN license in State of employment required.
• Minimum of 2 years clinical work experience required.
• Case management certification preferred.

Responsibilities

• Coordinates integration of social services/case management with discharge planning and home care.
• Conducts concurrent medical record review using approved indicators.
• Acts as patient advocate and educates staff on resource utilization and discharge planning.
• Promotes efficient utilization of clinical resources to achieve outcomes within timeframes.
• Reviews service utilization from admission to discharge.
• Evaluates patient satisfaction and quality of care.
• Communicates with physicians regularly to develop effective working relationships.
• Assists physicians in maintaining costs and patient outcomes.
