Qualifications

Responsibilities

• Act as a primary case manager to 3-6 consumers.
• Provide nursing interventions to individuals.
• Facilitate and coordinate medical care.
• Provide health education to individuals.
• Assist in medication management.
• Update comprehensive assessments.
• Transport consumers as needed.
• Provide 24/7 support through on-call coverage.

Skills
