Responsibilities
- Act as a primary case manager to 3-6 consumers.
- Provide nursing interventions to individuals.
- Facilitate and coordinate medical care.
- Provide health education to individuals.
- Assist in the management of medications and the department.
- Formulate and update Comprehensive Assessments and IAPs.
- Transport consumers as needed.
- Participate in rotating on-call coverage and evening shifts.
