Eliot Community Human Services, Inc. is seeking a licensed RN to join our PACT team in Massachusetts as a Nurse Case Manager. The role emphasizes outreach, coordinated care, and home-based support for adults with serious mental illness.

Responsibilities include primary case management for 3-6 clients, coordinating medical care with providers, health education, and medication management, with rotating on-call coverage for 24/7 support. Salary range $90,000–$100,000 USD.

Qualifikationen

• Requires an associates degree in nursing.
• LPN or RN nursing license.
• Experience working with adults with mental illness and knowledge of psychiatric and medical medications.

Aufgaben

• Act as a primary case manager to 3-6 consumers
• Provide nursing interventions to individuals
• Facilitate and coordinate medical care to include communication with health care providers
• Provide health education to individuals
• Assist in the management of medications and the overall functioning of the nursing department
• Formulate and regularly update Comprehensive Assessments and IAP's for clients on your caseload
• Transport consumers to work sites, meetings, daily activities, appointments, and activities as needed.
• Participate in rotating on-call coverage and evening shifts to ensure 24/7 support for clients
