Job summary

Eliot Community Human Services, Inc. seeks a licensed RN, Nurse Case Manager to join the PACT team and deliver comprehensive treatment, rehabilitation, and support to adults with serious mental illness. You will work in outreach, case management, and home-based support, coordinating care with health providers.

The role offers a Monday–Friday schedule with on-call and rotating evenings, a $5,000 sign-on bonus, and generous benefits to support growth and well-being.

Qualifications

• Associate degree in nursing required.
• LPN or RN nursing license.
• Experience with adults with mental illness and knowledge of psychiatric/medical meds.
• Massachusetts driver’s license.
• Schedule includes Monday–Friday with rotating evenings and one weekend per month.
• Bilingual candidates fluent in a needed language may receive a 6% salary increase.

Responsibilities

• Act as a primary case manager to 3-6 consumers.
• Provide nursing interventions to individuals.
• Coordinate medical care with health care providers.
• Provide health education to individuals.
• Assist in managing medications and nursing department functions.
• Formulate and update Comprehensive Assessments and IAPs for clients on caseload.
• Transport clients to work sites, meetings, and activities as needed.
• Participate in rotating on-call coverage and evening shifts for 24/7 support.
