Responsibilities

• Act as a primary case manager to 3-6 consumers.
• Provide nursing interventions to individuals.
• Facilitate and coordinate medical care.
• Provide health education to individuals.
• Manage medications and nursing department functioning.
• Update Comprehensive Assessments and IAPs for clients.
• Transport consumers to various appointments.
• Ensure 24/7 support through rotating shifts.
