Job summary

Elwyn seeks a Registered Nurse (RN) to join the Program for Assertive Community Treatment (PACT) in Massachusetts. The RN will assess, plan, coordinate, and deliver treatment, rehabilitation, and support services to clients in the community under supervision of the team leader and prescriber.

The role emphasizes person-centered care, medication management, and collaboration with a multidisciplinary team to support recovery and independence.

Qualifications

• Associate's degree in nursing is required; a Bachelor's is preferred.
• 3+ years in leadership/management roles with behavioral health experience preferred.
• Massachusetts RN license required; must meet MA regulatory standards.
• Cultural and linguistic needs of the area should be considered in care.

Responsibilities

• Provide nursing supervision to the team as assigned.
• Administer medications and manage medication policies under team prescriber.
• Coordinate health care and medical services for PACT clients in the community.
• Develop and implement the Community Service Plan with the ITT; provide therapy as needed.
• Educate clients and families on treatment plans, health needs, and medication use.
• Maintain electronic health records and document client progress and outcomes.
