Job summary
Eliot Community Human Services in Boston is seeking a licensed Nurse Case Manager to join our Program for Assertive Community Treatment team. Qualifications
- Requires knowledge of psychiatric and medical treatment.
- Experience working with adults with mental illness.
- Valid Massachusetts driver's license required.
Responsibilities
- Act as a primary case manager to 3-6 consumers.
- Provide nursing interventions to individuals.
- Facilitate and coordinate medical care.
- Provide health education to individuals.
- Assist in the management of medications.
- Formulate and regularly update assessments.
- Transport consumers to various activities.
- Participate in rotating on-call coverage.
