A community health organization in Massachusetts is seeking a skilled Nurse Case Manager to join their Program for Assertive Community Treatment team. The ideal candidate will have nursing expertise, particularly with psychiatric care. Responsibilities include case management, providing nursing interventions, and health education for individuals with severe mental illness. This full-time role includes a $5,000 sign-on bonus and a competitive salary of $77,680 USD.

Formación

• Requires an associates degree in nursing.
• LPN or RN nursing license needed.
• Experience working with adults with mental illness and knowledge of psychiatric and medical medications.

Responsabilidades

• Act as a primary case manager to 3-6 consumers.
• Provide nursing interventions to individuals.
• Facilitate and coordinate medical care.
• Provide health education to individuals.
• Assist in the management of medications.
• Formulate and regularly update Comprehensive Assessments.
• Transport consumers as needed.
• Participate in rotating on-call coverage.

Conocimientos

Experience working with adults with mental illness

Knowledge of psychiatric and medical medications

Educación

Associates degree in nursing

LPN or RN nursing license
