Job summary

Eliot Community Human Services, Inc. is seeking a licensed RN as a Nurse Case Manager to join the PACT team in Massachusetts. The role involves direct patient care, case management, medication management, and coordination with health providers in community settings.

The position offers a Monday–Friday schedule with rotating shifts and weekend coverage, with a salary range of $90,000 to $100,000 USD, plus comprehensive benefits and 401(k) matching.

Qualifications

• Requires RN license and experience with adults with mental illness.
• Experience in case management and care coordination.
• Knowledge of psychiatric and medical medications is preferred.

Responsibilities

• Act as primary case manager to 3-6 consumers.
• Provide nursing interventions to individuals.
• Coordinate medical care and communicate with providers.
• Deliver health education to individuals.
• Assist in medication management and nursing department operations.
• Develop and update Comprehensive Assessments and IAPs.
