Job summary

AltaMed Health Services is seeking a PACE Social Work Supervisor to oversee the social work team at the assigned PACE site. You will supervise master-level social workers and coordinate care planning with an interdisciplinary team to deliver person-centered care.

Responsibilities include guiding behavioral health discussions, life planning, and ensuring regulatory compliance while supporting onboarding and performance evaluations.

Qualifications

• Master of Social Work (MSW) from an accredited university is required.
• LCSW is preferred.
• Minimum 2 years in a supervisory role in health or human services.

Responsibilities

• Provide administrative oversight of the Social Work team at the PACE site.
• Coordinate routine assessments and care plans with interdisciplinary staff.
• Support family conferences, referrals, and ensure compliant documentation.
