Job summary

Midland Care Connection is seeking an RN Case Manager to coordinate and oversee comprehensive home-based care for older adults in the Kansas City area. You will assess health status, develop care plans, and coordinate services across an interdisciplinary care team to support independence and quality of life at home.

The role requires an active RN license, a strong background in geriatric care, and the ability to travel to participant homes and care sites.

Qualifications

• Active RN license in good standing.
• Graduated from an accredited nursing program.
• Valid driver’s license and ability to travel to participant homes and care sites.
• Strong assessment, care coordination, and documentation skills.
• Ability to work collaboratively with an interdisciplinary care team.
• Knowledge of chronic disease management and care of older adults.

Responsibilities
