Job summary

Midland Care Connection in Topeka, KS seeks an RN Case Manager to coordinate comprehensive in-home care for older adults enrolled in the PACE program, helping them remain safely at home with dignity.

The role includes assessing health status, creating care plans, coordinating services with an interdisciplinary team, and monitoring medical, functional, and psychosocial needs to ensure continuity and high quality of life.

Qualifications

• Active RN license and graduation from an accredited nursing program.
• Valid driver's license and ability to travel to participant homes and care sites.
• Strong assessment, care coordination, and documentation skills.
• Ability to work collaboratively with an interdisciplinary care team.
• Knowledge of chronic disease management and geriatric care.

Responsibilities

• Coordinate comprehensive care for older adults in the PACE program.
• Support and advocate for participants and families to promote independence.
• Monitor health status and adjust care plans as needed.
