Job summary

Midland Care Connection is seeking an RN Case Manager for its PACE program in the Topeka area. The role focuses on coordinating comprehensive care for older adults who prefer to remain at home, with plans tailored to complex medical needs.

You will collaborate with an interdisciplinary team, monitor medical, functional, and psychosocial needs, and advocate for participants and families to promote independence and safety.

Qualifications

• Active RN license and good standing.

It provides complete medical care, rehabilitation, medications, transportation, social services, and supportive therapies—all coordinated by an interdisciplinary care team. Participants receive personalized care plans tailored to their unique needs, along with access to a PACE center for social activities, meals, and clinical services. By integrating medical and supportive care under one program, PACE helps seniors maintain independence, improve quality of life, and continue living in their communities with dignity.RN Case ManagerCoordinates and oversees comprehensive care for older adults with complex medical needs, helping them remain safely in their homes and communities. This role involves assessing participants’ health status, developing and updating individualized care plans, coordinating services across an interdisciplinary team, and monitoring ongoing medical, functional, and psychosocial needs.
