Midland Care Connection seeks a dedicated RN Case Manager to coordinate patient-centered end-of-life care in the Kansas City area. You will assess patient needs, develop care plans, and ensure symptom management and comfort while collaborating with physicians, social workers, chaplains, and volunteers.

You will educate families, support decision-making, and advocate for patients’ wishes, ensuring regulatory compliance and high-quality hospice services across the care team.

Qualifications

• Current and valid Registered Nurse (RN) license in the state of practice.
• Minimum of 2 years of nursing experience, preferably in home health or hospice.
• Strong assessment and clinical decision-making skills.
• Excellent communication and interpersonal skills for interacting with patients, families, and interdisciplinary teams.

Responsabilités

• Coordinate and deliver high-quality, patient-centered end-of-life care.
• Develop individualized care plans and ensure symptom management and comfort.
• Act as the primary clinical liaison between patients, families, and the hospice team.
• Provide education and emotional support to families, navigating complex medical decisions.
• Ensure regulatory compliance and advocate for the patient’s wishes and quality of life.
