Midland Care Connection Inc. is seeking a dedicated RN Case Manager to coordinate care for older adults in their homes and communities. The role supports participants within the PACE program, developing care plans and coordinating services with an interdisciplinary team.

The ideal candidate is an active RN with strong assessment, documentation, and communication skills, able to travel to participant homes and care sites in the area.

Qualifications

• Active Registered Nurse license.
• Strong assessment and documentation skills.
• Ability to work collaboratively with an interdisciplinary care team.
• Familiarity with chronic disease management in older adults.

Responsibilities

• Coordinate and oversee comprehensive care for older adults in their homes and communities.
• Develop and update individualized care plans in collaboration with the care team.
• Act as clinical resource and advocate for participants and families, ensuring holistic, continuous care.
