Job summary

Midland Care Connection in Topeka, KS seeks an RN Case Manager to coordinate and oversee comprehensive care for older adults with complex medical needs, ensuring services are delivered safely in home and community settings.

You will assess health status, develop individualized care plans, and collaborate with an interdisciplinary team while guiding families to access PACE resources and maintain quality of life.

Qualifications

• Active RN license.
• Graduation from an accredited nursing program.
• Valid driver’s license and ability to travel to participant homes and care sites.
• Strong assessment, care coordination, and documentation skills.
• Ability to work collaboratively with an interdisciplinary care team.
• Knowledge of chronic disease management and care of older adults.

Responsibilities

• Coordinate and oversee comprehensive care for older adults with complex medical needs, helping them remain safely in their homes and communities.
• Assess participants’ health status, develop and update individualized care plans, and coordinate services across an interdisciplinary team.
• Monitor ongoing medical, functional, and psychosocial needs and advocate for participants and families to ensure holistic, continuous care aligned with PACE standards.
