Job summary

Midland Care Connection is seeking an RN Case Manager for its PACE program in the Topeka area. The role focuses on coordinating comprehensive care for older adults who prefer to remain at home, with plans tailored to complex medical needs.

You will collaborate with an interdisciplinary team, monitor medical, functional, and psychosocial needs, and advocate for participants and families to promote independence and safety.

Qualifications

• Active RN license and good standing.
• RNs with BSN or equivalent preferred.
• Ability to travel to participant homes and care sites.

Responsibilities

• Coordinate and oversee comprehensive care for older adults in their homes.
• Develop and update individualized care plans.
• Coordinate services across an interdisciplinary team and monitor needs.
• Advocate for participants and families to ensure quality of life.
