Midland Care Connection in Topeka, Kansas, is seeking an experienced RN Case Manager to coordinate and oversee care for older adults with complex needs, helping them stay safely in their homes and communities.

As part of the PACE program, you will assess health status, develop care plans, and collaborate with an interdisciplinary team to ensure holistic, continuous, and person-centered care.

Qualifications

• Active RN license.
• Graduation from an accredited nursing program.
• Valid driver’s license and ability to travel to participant homes and care sites.
• Strong assessment, care coordination, and documentation skills.
• Ability to work collaboratively with an interdisciplinary care team.
• Knowledge of chronic disease management and care of older adults.

Responsabilités

• Coordinate and oversee comprehensive care for older adults to remain safely in their homes and communities.
• Assess participants’ health status and develop individualized care plans.
• Coordinate services across an interdisciplinary team to ensure holistic care.
• Monitor ongoing medical, functional, and psychosocial needs; advocate for participants and families.
