Job summary

Habitat Health in California is seeking an Intake Coordinator to facilitate the enrollment process for prospective participants into our PACE program. You will coordinate home visits, center tours, assessments, and follow-up appointments while partnering with caregivers and cross-functional teams to ensure a smooth, participant-centered experience.

Responsibilities include documenting in CRM/EMR, maintaining timely communication with Outreach & Enrollment and clinical teams, and guiding

Qualifications

• Bachelor's degree or equivalent in healthcare or related field.
• 1+ year experience in healthcare enrollment or care coordination.
• Experience with Medicare/Medicaid eligibility or enrollment preferred.

Responsibilities

• Coordinate end-to-end intake for prospective participants with cross-functional teams.
• Schedule home visits, center tours, assessments, and follow-up visits.
• Maintain proactive communication with Outreach & Enrollment and clinical teams.
• Coordinate with clinical teams for additional assessments to support eligibility.
• Guide participants and caregivers through center tours and education on the PACE model.
• Document participant information and activity in CRM/EMR in a timely manner.
• Track requirements to ensure timely documentation and enrollment progress.
• Support participant handoffs from intake through enrollment and into IDT.
• Assist with community events and enrollment conferences.
