Job summary

Habitat Health is seeking an Intake Coordinator in California to manage the end-to-end intake and enrollment for potential participants in the PACE program. You will coordinate home visits, tours, assessments, and follow-up activities across clinical and outreach teams to ensure smooth enrollment and a strong participant experience.

Qualifications

• Bachelor's degree in Healthcare Administration, Business Administration, Marketing, or a related field.
• 1 year of experience in healthcare, enrollment, participant services, care coordination, or a similar role.
• Experience with Medicare/Medicaid or eligibility programs is preferred.

Responsibilities

• Coordinate end-to-end intake for prospective participants with cross-functional teams to enable timely enrollment.
• Schedule home visits, center tours, assessments, and follow-ups to maintain progress in intake and enrollment.
• Maintain proactive communication with outreach, enrollment, and clinical teams for timely updates and efficient coordination.
• Assist clinical teams to arrange additional assessments to determine eligibility and IDT presentations.
• Guide prospective participants and caregivers through education about the PACE model and Habitat Health services.
