Job summary

Habitat Health in Sacramento, CA is hiring a Medical Assistant to support delivery of high-quality, participant-centered care within the PACE center. You will work closely with providers and the IDT to manage intake, vital signs, and routine procedures, while maintaining accurate EMR documentation and coordinating care activities.

The role emphasizes collaboration, efficient patient flow, and adherence to safety and infection control protocols in a fast-paced environment.

Qualifications

• Current phlebotomy certification.
• High school diploma or equivalent; completion of an accredited Medical Assistant program.
• Current Medical Assistant certification.
• Ability to perform routine clinical procedures within scope of practice.
• Strong communication and collaboration skills for care coordination.
• Excellent organizational skills and ability to manage multiple priorities.
• Experience using EMRs and ability to learn systems quickly.

Responsibilities

• Support intake, rooming, vital signs, screenings, and provider orders.
• Perform laboratory testing, phlebotomy, EKGs, immunizations, injections, CGM.
• Communicate observations and participant needs to the care team for coordinated care.
• Coordinate follow-up visits and assessments with the care team.
• Serve as first contact for participant questions via phone or email.
• Maintain accurate intake and clinical documentation in EMR and systems.
• Assist with medical record management, referrals, and authorizations.
• Ensure clinic readiness, stock exam rooms, and manage equipment.
