Job summary

Habitat Health in the United States is seeking a Medical Assistant to support high-quality, participant-centered care within the PACE program. You will work with providers and the IDT to support participant flow, perform routine clinical tasks, document accurately, and maintain a welcoming care environment.

The role requires a high school diploma or MA program, MA certification, phlebotomy certification, EMR proficiency, and the ability to travel between sites.

Qualifications

• High school diploma or equivalent; completion of an accredited Medical Assistant program.
• Current Medical Assistant certification or credential as required by state law.
• Current phlebotomy certification.
• Ability to perform routine clinical procedures within scope of practice.
• Strong communication and collaboration skills with the Interdisciplinary Team.

Responsibilities

• Support participant flow by intake, vitals, and preparing for exams.
• Perform phlebotomy, EKGs, immunizations, and other routine clinical tasks.
• Communicate observations and changes in status to support coordinated care.
• Coordinate follow-up visits and appointments with the care team.
• Maintain accurate EMR documentation and administrative tasks.
