Job summary

Habitat Health is seeking a Medical Assistant to support the PACE center with clinical and administrative duties. You will assist with intake, vital signs, exams, and routine procedures, working closely with providers and the Interdisciplinary Team to ensure smooth participant flow.

Qualifications

• Completion of an accredited Medical Assistant program.
• Current Medical Assistant certification and phlebotomy certification.
• Experience with EMRs and clinical documentation.
• Ability to perform routine clinical procedures within scope of practice.
• Strong communication and teamwork in a fast-paced clinic.

Responsibilities

• Complete intake, rooming, vital signs, and prepare for exams.
• Perform phlebotomy, EKGs, immunizations and injections.
• Coordinate with IDT to support coordinated care.
• Schedule follow-up visits and coordinate with care team.
• Serve as first point of contact for participant questions and needs.
• Maintain accurate EMR documentation and records.
• Assist with referrals, records, and authorizations.
• Maintain clinic readiness and inventory.
