Job summary

Habitat Health is expanding its PACE program and seeks a Medical Assistant to support high-quality, participant-centered care in the center and at home. You will work with providers and the IDT to manage intake, vitals, procedures, and documentation, while ensuring smooth participant flow and safety.

The role requires MA certification, phlebotomy credentials, and EMR experience, with a commitment to teamwork, quality, and continuous improvement in a fast-paced clinical setting.

Qualifications

• High school diploma or equivalent; completion of an accredited Medical Assistant program.
• Current Medical Assistant certification or credential as required by state law.
• Current phlebotomy certification.
• Experience using EMRs and documenting clinical data.

Responsibilities

• Support participant flow by intake, rooming, vital signs, and exam prep.
• Perform laboratory testing, phlebotomy, EKGs, immunizations, injections, and glucose monitoring.
• Coordinate follow-up visits and external appointments with the care team.
• Maintain accurate intake and clinical documentation in the EMR.
• Provide welcoming, participant-centered care and respond to questions or escalations.
• Assist with referral tracking, medical record management, and authorizations.
