Job summary

Habitat Health is seeking a Medical Assistant to support care within our PACE center in California. You will work with the IDT to assist with intake, vital signs, and routine clinical procedures, while maintaining accurate documentation in our EMR.

The role emphasizes patient-centered care, collaboration, and efficiency in a fast-paced setting. A current MA certification, phlebotomy credential, and CPR/BLS are required or obtainable within 30 days.

Qualifications

• High school diploma or equivalent and accredited Medical Assistant program completed.
• Current Medical Assistant certification or credential as required by state law.
• Current phlebotomy certification.
• Experience using EMR systems and clinical documentation.
• Strong communication and collaboration with participants, caregivers and the IDT.
• Strong organizational skills and ability to manage multiple priorities.
• Active CPR/BLS certification or ability to obtain within 30 days of hire.
• Meets health requirements including TB clearance and immunizations.

Responsibilities

• Support participant flow by intake, rooming, vital signs and exam prep.
• Perform lab testing, phlebotomy, EKGs, immunizations and injections.
• Communicate observations and needs to the IDT for coordinated care.
• Coordinate follow-up visits and appointments with the care team.
• Serve as first contact for participant questions via phone and electronic channels.
• Maintain accurate intake and clinical documentation in the EMR.
• Support referrals, medical records and authorizations to enable timely care.
• Maintain clinic readiness and manage supplies and inventory.
• Provide a welcoming, participant-centered experience.
