Job summary

Habitat Health is seeking a Medical Assistant to support the PACE program, delivering clinical and administrative support in the center and at participant homes. This role partners with providers and the IDT to ensure smooth participant flow, accurate documentation, and timely care.

Responsibilities include intake, vital signs, phlebotomy, immunizations, and coordinating follow-up visits within a fast-paced environment.

Qualifications

• Current phlebotomy certification.
• Completion of an accredited Medical Assistant program.
• Current Medical Assistant certification.
• Ability to perform routine clinical procedures (intake, vital signs, screenings, specimen collection).
• Strong communication and collaboration with participants, caregivers, and IDT.

Responsibilities

• Manage participant intake, vitals, and screenings.
• Perform phlebotomy, EKGs, immunizations, injections, and other routine procedures.
• Coordinate follow-up visits and external appointments with care team.
• Maintain accurate EMR documentation and records.
• Serve as initial contact for participant questions and care-related requests.
