Job summary

Habitat Health is actively seeking a Medical Assistant to support the PACE center with clinical and administrative tasks. You will work closely with providers and the Interdisciplinary Team (IDT) to support participant flow, documentation, and routine procedures within scope of practice.

Strong communication, collaboration, and organizational skills are essential in a fast-paced setting. EMR experience and CPR/BLS are advantageous, with bilingual abilities a plus.

Qualifications

• High school diploma or equivalent; completion of an Accredited Medical Assistant program.
• Current Medical Assistant certification or credential as required by applicable state law.
• Current phlebotomy certification.

Responsibilities

• Support efficient participant flow by completing intake, rooming, vitals, and preparing for examinations.
• Perform routine clinical procedures: phlebotomy, EKGs, immunizations, injections, and related tasks.
• Communicate observations and changes in status to the care team to support coordinated care.
• Coordinate follow-up visits and other clinical activities with the care team.
