Job summary

Midland Care Connection, a not-for-profit organization serving older adults and seriously ill individuals in northeast Kansas, seeks a PACE Intake Coordinator. You will guide potential participants and families through intake, collect necessary documents, and coordinate with the interdisciplinary team to assess eligibility and enrollment readiness.

Established in 1978, we offer a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and our signature PACE (Program of All-Inclusive Care for the Elderly) program. PACE Program

The PACE (Program of All-Inclusive Care for the Elderly) program is designed to help older adults remain safely in their homes rather than entering nursing facilities. It provides complete medical care, rehabilitation, medications, transportation, social services, and supportive therapies—all coordinated by an interdisciplinary care team. Participants receive personalized care plans tailored to their unique needs, along with access to a PACE center for social activities, meals, and clinical services. By integrating medical and supportive care under one program, PACE helps seniors maintain independence, improve quality of life, and continue living in their communities with dignity.

PACE Intake Coordinator

The PACE Intake Coordinator is often the first point of contact for individuals interested in joining the PACE program. They help guide potential participants and their families through the intake process, gather necessary information and documentation, coordinate assessments and appointments, and work closely with the interdisciplinary team to determine eligibility and readiness for enrollment. They also help ensure the enrollment process is organized, timely, and compliant with PACE requirements while providing a positive experience for participants and their families.

Qualifications
Required
  • High school diploma or equivalent
  • At least 1–2 years of experience in healthcare, social services, customer service, admissions/intake, or a related administrative role
  • Strong verbal and written communication skills
  • Excellent organizational skills and attention to detail
  • Ability to manage multiple tasks, prioritize responsibilities, and meet deadlines
  • Strong customer service skills and the ability to interact professionally with participants, families, caregivers, and community partners
  • Proficiency with Microsoft Office and the ability to learn electronic health records and other computer systems
  • Ability to maintain confidentiality and handle sensitive information appropriately
  • Ability to work independently while also collaborating effectively with an interdisciplinary team
Preferred
  • Associate degree in healthcare administration, business, social services, human services, or a related field
  • Previous experience working with older adults or individuals with disabilities
  • Experience in a PACE program, healthcare organization, senior services, or social-service agency
  • Experience with participant intake, admissions, referrals, enrollment, or care coordination
  • Familiarity with Medicare, Medicaid, or other healthcare programs
  • Knowledge of local community resources and services for older adults
  • Experience using an electronic medical record (EMR/EHR) or similar database system
