Job summary

Habitat Health is seeking an Intake Coordinator to facilitate the enrollment process for participants in its PACE program. You will coordinate home visits, assessments, and center tours, while partnering with caregivers and cross-functional teams to ensure a smooth, participant-centered intake experience.

You will maintain accurate participant information in CRM/EMR systems and guide families through enrollment with education on Habitat Health’s services and the PACE model.

Qualifications

• Bachelor's degree in Healthcare Administration, Business Administration, Marketing, or a related field, or an equivalent combination of education and relevant experience.
• Minimum 1 year experience in healthcare, enrollment, participant services, care coordination, or similar roles.
• Experience using CRM/EMR systems and MS Office.
• Strong communication and collaboration skills with cross-functional teams.

Responsibilities

• Coordinate end-to-end intake for prospective participants to support timely enrollment.
• Schedule and conduct home visits, center tours, and assessments; manage follow-up appointments.
• Maintain proactive communication with Outreach, Enrollment, and clinical pre-enrollment teams.
• Facilitate additional assessments with clinical teams to support eligibility and IDT presentations.
• Guide participants and caregivers through tours and education on the PACE model and Habitat Health services.
• Maintain accurate participant data and documentation in CRM/EMR systems.
