Midland Care Connection, a not-for-profit organization serving older adults and seriously ill individuals in northeast Kansas, seeks a PACE Intake Coordinator. You will guide potential participants and families through intake, collect necessary documents, and coordinate with the interdisciplinary team to assess eligibility and enrollment readiness.

The role requires strong communication, organizational abilities, and experience with healthcare or social services intake.

Qualifications

• High school diploma or equivalent is required.
• 1–2 years in healthcare, social services, customer service, admissions/intake, or related administrative role preferred.
• Strong verbal and written communication skills; attentive to detail and deadlines.

Responsibilities

• Be the first point of contact for potential PACE participants and families.
• Coordinate intake steps, gather required information, and schedule assessments.
• Collaborate with the interdisciplinary team to determine eligibility and enrollment readiness.
• Ensure enrollment processes are organized, timely, and compliant with PACE requirements.
