Job summary

Golden Valley Health Centers in Merced, CA, seeks a PACE IDT Coordinator to assist with and coordinate administrative and operational activities for the Central Valley PACE IDT. The role interfaces with IDT members, participants, and caregivers to document issues and resolve needs.

Responsibilities include scheduling, care plan management, and contributions to audits and data reporting. Strong organizing, communication, and healthcare experience are essential; bilingual English/Spanish a plus.

Qualifications

• High school diploma or equivalent.
• Two years of experience in process administration, quality, compliance, or direct patient care.
• One year of experience in a clinical healthcare setting.
• Familiarity or past coding experience.
• PACE experience preferred.

Responsibilities

• Coordinate administrative and operational activities for the Central Valley PACE IDT.
• Maintain the care plan calendar and inform of due assessments.
• Audit IDT tasks for compliance with Health Plan and Quality Improvement teams.
• Monitor provider documentation for HCC coding opportunities and provide feedback.
• Provide administrative support to the IDT (scheduling, meetings, inquiries).
• Prepare and distribute binders, reports, agendas, and minutes of IDT meetings.
