Job summary

Golden Valley Health Centers in Modesto, CA is seeking a PACE Primary Care Physician to provide direct primary medical care to PACE participants and supervise IDT staff. The role requires leadership within a multidisciplinary team and flexible hours.

Responsibilities include comprehensive histories, physicals, care coordination, and acting as attending physician for hospitalizations, with emphasis on geriatric care and home-based evaluations.

Qualifications

• Graduate of an accredited medical school with MD/DO degree.
• Licensed in California with board-eligibility or certification in Family Practice or Internal Medicine.
• Two years of clinical experience; geriatric care experience preferred.
• Bilingual English/Spanish is highly preferred.
• Current CPR certification and DEA registration.

Responsibilities

• Perform comprehensive histories and physicals on new referrals.
• Lead and coordinate care within the IDT for PACE participants.
• Admit and coordinate hospital care when indicated.
• Oversee care of participants in nursing homes and during discharges.
