Job summary

Golden Valley Health Centers is seeking a PACE Primary Care Physician to provide direct medical care to aging participants at the Central Valley Merced PACE Center. You will lead an interdisciplinary team, supervising NPs, PAs, RNs, and aides to ensure high-quality, person-centered care within the PACE program.

The role requires California medical licensure, board eligibility/certification in Family Practice or Internal Medicine, and eligibility to work in the United States.

Qualifications

• MD/DO medical degree from an accredited school.
• California medical license in good standing.
• Current American Heart Association BLS CPR certification.
• DEA certificate required.
• Minimum two years of clinical experience.
• Fellowship in geriatrics preferred.

Responsibilities

• Perform comprehensive history and physicals on new referrals.
• Provide ongoing care for participants and coordinate with the IDT.
• Admit participants to hospital when indicated and coordinate discharge planning.
• Order, interpret and follow up on labs and tests; document in EMR.
• Participate in ethics, quality assurance, and staff meetings.
• Precept medical students and residents during PACE rotation.
