Job summary

Neighborhood Healthcare is seeking a PACE Center Operations Coordinator in Riverside, CA to support IDT processes and center operations. You will manage schedules, care planning materials, and documentation to ensure compliant, efficient workflows on-site.

Responsibilities include coordinating IDT meetings, tracking care plans, and assisting with patient transitions while maintaining strong communication with participants and caregivers.

Qualifications

• High School Diploma or GED required; BLS cert required on hire.
• CPR and First Aid certification required for all PACE employees.
• Minimum 2 years of administrative support experience.
• Strong communication, organization and multitasking abilities.

Responsibilities

• Coordinate Interdisciplinary Team (IDT) processes and schedules to ensure timely care planning.
• Prepare IDT agendas, materials, and serve as scribe for meetings.
• Maintain IDT tracking tools for care plans and assessments.
• Follow up on IDT action items and ensure timely completion.
• Track transitions of care and communicate updates to the team.
• Support documentation and compliance with medical records and quality teams.
• Schedule meetings, arrange catering and travel as needed; record minutes.
• Provide front desk coverage and assist with checks-in/out during peak times.
• Coordinate site operations and participate in policy/procedure implementation.
