Job summary

High Desert PACE seeks a Care Plan Coordinator to oversee the comprehensive lifecycle of care plan activities under the Chief Clinical Officer. You will supervise Interdisciplinary Team (IDT) members and optimize service delivery for integrated, high-quality participant care.

Responsibilities include leading Care Planning meetings, ensuring timely documentation, coordinating with home care, nursing, social work, therapy, and other staff, and maintaining compliance with federal and state

Qualifications

• 2+ years of supervisory or clinical administrative experience in health care.
• 1+ year of experience with frail or elderly population.
• Must be medically cleared and up-to-date immunizations.

Responsibilities

• Participate as a member of the interdisciplinary team and facilitate Care Planning meetings.
• Oversee Care Planning activities and ensure timely communication across departments.
• Communicate with IDT staff including nurses, social workers, therapists, and coordinators.
• Assist with policy development and adherence for Care Planning activities.
