Job summary

Golden Valley Health Centers seeks an Intake Specialist to manage enrollment for the PACE program, educate participants and caregivers, gather information, set assessments, and document needs for new entrants.

This full-time role requires bilingual English/Spanish communication, a CA driver's license, and proficiency with Microsoft Office; training will occur in Modesto before Stockton deployment.

Qualifications

• Must be bilingual in English and Spanish.
• Strong communication and presentation skills, both oral and written.
• Proficiency with computer Microsoft Office suite.

Responsibilities

• Provides courteous customer service in person and over the phone and educates callers about available services.
• Conducts program eligibility screening, intake, education and enrollment assistance.
• Documents medical and social indications for enrollment and coordinates with the interdisciplinary team.
