Job summary

Central Valley PACE in Merced, CA is seeking a PACE Interdisciplinary Team (IDT) Coordinator to support administrative and operational activities for the IDT. You will help develop care plans, enroll participants, coordinate assessments, and document issues, interfacing with participants, caregivers, and IDT members.

The role requires strong data management, communication, and problem-solving skills, with daily 8:00am–5:00pm hours and on-site work at the Merced center.

Qualifications

• High school diploma or equivalent required; two years admin/quality/patient care experience preferred.
• Experience in clinical healthcare settings is desirable.
• Bilingual English/Spanish preferred; strong written and verbal communication skills.

Responsibilities

• Supports IDT using organizational policies and procedures.
• Communicates participant status changes to vendors (pharmacies, respiratory care).
• Maintains care plan calendar and notifies due assessments.
• Performs audits with Health Plan and Quality Improvement teams for compliance.
• Monitors provider documentation for HCC coding opportunities and provides feedback.
