Job summary

United Health Centers of the San Joaquin Valley seeks a Full Time Intake and Enrollment Specialist for PACE. You will guide prospective participants and caregivers through enrollment, coordinate assessments, and support completion of enrollment documents.

You will work with a multidisciplinary team, ensure HIPAA compliance, and participate in outreach efforts to promote center resources for a diverse population.

Qualifications

• CPR certification (BLS or ACLS) required.
• Valid California Driver’s License required.
• 2+ years in healthcare, senior services, or enrollment coordination.
• 1 year with geriatric or frail populations.

Responsibilities

• Conduct initial screenings and determine program eligibility.
• Explain enrollment process and eligibility requirements.
• Assist with enrollment applications and required documentation.
• Schedule home visits, center tours, and team evaluations.
• Maintain HIPAA-compliant records and accurate documentation.
• Collaborate with marketing, outreach, and clinical teams to meet enrollment goals.
