Talent Connect Executive Search & HR Solutions is seeking a PACE Enrollment Specialist to support a San Fernando Valley healthcare organization. The role emphasizes outreach, enrollment, and relationship-building with seniors, families, and referral partners.

The candidate will educate about PACE, generate leads, and guide eligible individuals through enrollment, partnering with an interdisciplinary team and ensuring compliant activities. Bilingual English/Spanish is required.

Qualifications

• Bilingual English/Spanish required.
• Live in or near San Fernando Valley with strong local familiarity.
• Bachelor’s degree preferred; equivalent relevant experience considered.
• Minimum 2 years of experience in field sales, healthcare marketing, community outreach, enrollment, or related area.

Responsibilities

• Conduct ongoing community outreach throughout the San Fernando Valley to raise awareness of PACE services.
• Build and maintain relationships with community organizations, senior resources, healthcare providers, referral partners, and local groups.
• Identify new referral sources and generate qualified prospective participant leads.
• Educate seniors, families, caregivers, and partners about PACE and eligibility.
• Follow up with prospective participants and families; maintain an active enrollment pipeline.
