Job summary

Habitat Health is seeking an Intake Coordinator to facilitate the intake process for prospective participants entering our PACE program. You will coordinate home visits, center tours, assessments, and enrollment steps, partnering with caregivers and cross-functional teams to ensure a smooth, participant-centered experience.

You'll maintain participant information in CRM/EMR, support documentation, and coordinate with Outreach, Enrollment, and clinical pre-enrollment teams to enable timely

Qualifications

• Bachelor's degree or equivalent combination of education and experience.
• Minimum 1 year healthcare enrollment or participant services experience.
• Strong ability to coordinate multiple priorities with attention to detail and timely follow-through.

Responsibilities

• Coordinate end-to-end intake for prospective participants with cross-functional teams.
• Schedule home visits, center tours, assessments, and follow-ups to move enrollment forward.
• Maintain proactive communication with Outreach, Enrollment and clinical teams for timely updates.
• Collaborate with clinical teams to arrange additional assessments for eligibility.
• Provide a welcoming, participant-centered experience during tours and education on PACE services.
• Maintain accurate participant information and intake documentation in CRM/EMR.
• Track participant requirements through each intake stage and ensure documentation is complete.
• Support smooth handoffs from intake through enrollment to IDT.
• Assist in community events and enrollment conferences to promote program awareness.
• Ensure compliance with federal/state regulations and Habitat Health policies.
