Job summary

Habitat Health is seeking an Enrollment RN to lead clinical enrollment during the PACE onboarding process in California. You will perform nursing, home safety, and eligibility assessments, educate prospective participants and caregivers, and collaborate with the IDT to support informed enrollment decisions.

Qualifications

• Associate's degree in Nursing required.
• Active, unrestricted RN license in the applicable state.
• Minimum 1 year of nursing experience with older adults or complex medical needs.
• Ability to conduct nursing/home safety assessments and support enrollment decisions.
• Strong organizational and communication skills; able to collaborate with cross-functional teams.
• Experience using electronic health records (EHRs) and related systems.

Responsibilities

• Partner with Outreach/Enrollment Specialists and IDT to ensure a coordinated enrollment experience.
• Conduct nursing, home safety, and eligibility assessments in participants' homes or care environments.
• Complete NFLOC assessments and DHCS-compliant documentation.
• Present clinical findings in enrollment eligibility meetings to support decisions.
• Build relationships with participants and caregivers, providing education and support.
• Maintain accurate clinical documentation in the EHR and related systems.
• Stay informed about PACE eligibility, enrollment processes, and regulatory requirements.
• Engage in outreach and community activities to support growth and enrollment.
